Hello, Rose. This additional feedback just came in from our medical expert who had addressed your original inquiry:

"The Tram flap should take care of her problem by providing soft tissue and releasing the painful scar and replacing some of the painful chest skin. She needs to show her surgeon where the pain is so that he/she can resect the
skin that is painful. This should work well for her."

He is a very well respected, top in his field, plastic surgeon. This sounds very encouraging and promising to me! I hope that this clarification is helpful.
